前 言
近期，发生许多船长写的海事声明和海事事故报告， 由于书写的格式不对受
到了主管机关的拒签， 造成了无法进行索赔， 同时也给海事事故处理带来了许多
麻烦，为了使船长今后规范书写海事声明和海事事故报告， 特将海事声明和海事
事故报告的标准格式发放给船长，请船长妥善保存、并列入交接。

定义
船舶在锚泊、停泊或航行时，遭遇到不可
控制的情况， 估计可能发生损坏或者损失
难以避免，但实情不详，为维护船舶利益，
船方向有关方提出保留索赔权力或免除
责任的书面声明。

中，由于事故引起的损害情
